Add a Bill

User Story

As a user I should be able to add a bill.

From the Profile Dashboard When a user clicks Add Bill They'll be taken to a form

Dev Notes

The form will include the following information:

• Provider Name
• Bill Date
• Bill Amount
• Account Number/Medical Billing Number
• Name on account
• The lowest monthly amount provider will accept as payment
• Provider Contact Phone number

At the bottom of the form will be a button that says Add Bill. When clicked, it'll add this bill to your user's list of bills along with updated your profile dashboard to show a new total bill amount and a new lowest monthly payment due.
