Are you using the HQ camera mod or any other camera?
What folder are the pics stored in? /sdcard/DCIM/...?

I have used the modded OneXed camera 3.0 and camera ICS+.

Pics are saved to /sdcard/DCIM/100MEDIA

and the pics show up on quickpic

The stock gallery uses the media database as opposed to some of the other
gallery apps which judt access the folders directly.

Try flashing the attached zip to delete the database and force Android to
rescan your SD card and recreate the media database.

The ringtones are still there, but I guess it's having a problem rescanning
your SD card.

Try fixing permissions via TWRP.

If that doesn't work, if you have adb/fastboot setup, try this:

1. boot up your phone
2. After the red text disclaimer on the boot screen, type "adb
logcat>c:\logcat.txt" from the Command Prompt (while in the same folder as
"adb.exe")
3. Let the phone boot up
4. Open the Gallery
5. Switch to the Command Prompt and press Control-C to stop it
6. Upload logcat.txt to Dropbox or some other service and send me the link
through PM

Other things to try:

Identify if your SD card is corrupt:
1. Take a backup of your data partition only
2. wipe data (factory reset) and reboot to see if your media gets picked up
3. If it does, the problem isn't with your SD card.
4. Restore your data

update. I have wiped data and everything is working again. I even installed
the oneXed camera and everything still works. It is very strange i cannot
flash the oneXed camera using TWRP and had to use root explorer to copy to
system/app, reboot and wipe dalvik and everything is working as it should.

I think the problem came from reflashing the rom a few times without wiping
data.
