Ability to configure the host part of the xml_cdr url

If your webserver is configured with virtual hosts, the url in 
autoload_configs/xml_cdr.conf.xml won't necessarily work. It would be nice if 
the hostname could be configured through FusionPBX instead of having to edit 
the freeswitch config file directly.

This also applies to the odbc dsn settings in lcr.conf.xml, switch.conf.xml and 
cidlookup.conf.xml, but I think that request has been made in another issue.

Best way to deal with these will be to use a freeswitch variable and then 
adjust from system -> variables.

I think I will default xml cdr to use the file system by default but possible 
to configure to use the POST. File system option because it will increase 
security and scalability.

${dsn} I have added to the dev branch but currently commented out in the xml 
because not everyone has dsn defined or is using odbc or native postgres option 
found in 1.2.7 and higher.

This request is more than 2 years old and I know for sure the CDRs are now 
stored in the DB. So we can mark this as fixed.

This request requires using a template engine for the configs. That is planned 
for the next major version of FusionPBX after the 3.8 release.
