Creating new domain in Multitenant setup #421

What steps will reproduce the problem?
1. Menu Advanced, Domains
2. Create new domain
3. Save

What is the expected output? What do you see instead?

Fatal error: Call to a member function fetchAll() on a non-object in 
/var/www/fusionpbx/includes/lib_switch.php on line 169

What version of the product are you using? On what operating system?
Version:    3.3
Operating System:   Linux pbxII 3.2.0-23-generic #36-Ubuntu SMP Tue Apr 10

I got this error as well when I was deleting a domain. I'm not quite sure how 
to get rid of this.

I too also got this issue today, 2nd Sep.

Version: 3.3 SVN Revision: 4427
FreeSWITCH Version 1.2.12+git~20130809T135531Z~ed45fbd4bd (git ed45fbd

OS: Ubuntu 12.04.3 LTS Linux 3.2.0-52-generic x86_64
PHP Version: PHP 5.3.10-1ubuntu3.7 with Suhosin-Patch (cli)

Tested on the development branch and haven't been able to duplicate this 
problem.
