Cannot label unlabelled buttons at the top of the screen (Action of Title bars?)

I don't know if it's a Title Bar or an Action Bar at the top of the screen but 
if there are unlabelled buttons to the right of the window name then I cannot 
add labels to them.

If I use the UP then RIGHT swipe to open the Local Context menu I hear "No menu 
items".

I'm running Android 4.4.2 on a Moto G.

An app which demonstrates this is TheTrainline. 

Another app is Realtime Trains and although it's buttons at the top are 
labelled if I try to change them again I get "No menu items" when I try to open 
the Local Context menu.
