Application Form

As a user I want to be able to submit an application for a job that I see advertised on the HMC form.

Acceptance Criteria

• [x] Design is complete for form. (Step 1 - Applicant Info)
• [ ] Inputs in the form should match design (i.e dropdown should be dropdowns)
• [x] User must have a view of completed form before submission
• [ ] Application is submitted and stored when user completes form.
