UI-2.2: Create an activity, that is linked from the main activity, that contains a form that will allow the user to create a watering schedule for his/her plants.

This issue is UI only. The form does not currently need to connect to anything. The form should validate user inputs.

Activity created but is a place holder. A new issue will be created for the form and hookup - closing
