[MOVE] move code to header

[hroest]

• templates need to be declared in the headers, otherwise IsoSpec cannot properly function as a library
• use C++11 feature #pragma once
• update licence to 2018
• add a ISOSPEC_ prefix to macros - this will help to incorporate IsoSpec into other peoples code
