Tondo says:

          Please use
                        \begin{theindex}
                        \addcontentsline{toc}{chapter}{\protect\numberline{}{Index}}
                         \markboth{INDEX}{INDEX}

Tondo's hint was almost right. Just had to get rid of the first line (\begin{theindex}) and it works well.
