Add "Is retired" and "Is preparatory" fields to profiles for Attribute Terminologies and Code Sets #81

Update the profiles so that the following are added:

Attribute Terminologies

Update the "Terminology" profile to include:

Attribute Terminologies - Terms

Update the "Term" profile to include:

Data Element Code Sets

Update the "CodeSet" profile to include:

Additional notes

These profile fields are common to other profiles, so copy from them for consistency.
