Global Table of Contents returning error #106

Hello,

When I try to use --globaltoc, any argument I pass in returns the error: "System.ArgumentNullException: Value cannot be null. (Parameter 'input')"

Looking at the code, I suspect it's looking for some kind of header and not finding it. How is this parameter meant to be used? My intent is to use the wiki page hierarchy as the table of contents, rather than individually specified TOC per page. Thanks for any information.
