--globaltoc with --heading #107

Hello, first off, thanks for this very useful tool which I have been experimenting with for a while. Currently able to output a DevOps Wiki as 8 separate PDFs (~ 400 pages) and really pleased with the results.

I'm using the --globaltoc and --heading parameters when combining a folder of wiki pages into a single PDF. One limitation is that the additional headings generated by --heading do not appear in the globaltoc. Is that intended and/or is there a way to get those headings into the toc? (I am not a developer and haven't looked into the code)
