Well done Jessie! Your work really captures the essence of the task. The form contains most of the relevant information needed to accurately record patients' symptoms. It will be great to design another version addressed to Health care professionals mostly to record retrospective data. Good job for using adobe XD for the design which offers a lot of flexibility in terms of visual background and features. It would have been perfect if it was also an open-source tool :) Well done too for having the design transferable to other platforms and collection tools. Nice Job!

Thanks. I'll work on another form to record retrospective data as well.
