Round out Minimized exclusion implementation + Exclusion

• Finishes the translation of MinimizedExclusions
• Replaces Tuple2<Organization, ModuleName> with a named Exclusion.

Note: I'm not convinced that even coursier has this right and its kinda hard to test until we are using it in a resolution.
