A few mods e.g. Smart Medicine stored settings for pawns, and were somehow getting these temp pawns stored. The save file would then have dangling refs to pawns that were not saved to the file.
For vanilla, I think occasionally some pawn relations might have been messed up, just logging warnings on load for the missing pawn.
