Description

Modify liquidity_pools table to include provider's address.

New columns to the table:

[x] Follow dbt style guide
[x] Tag the person(s) responsible for reviewing proposed changes @antonyip
[ ] Notes to deployment, if a full-refresh is needed for any table - I assume this requires some sort of refresh for the table.

Notes: There's a target/manifest.json that I didn't commit. I assumed this was meant to be gitignored.
