survey form is too narrow and does not resize properly on wider screens, it has scrolling issues also. #22

Our surveys are getting larger. I was testing one just now and could resize my browser but the form did not respond.

FYI - Feedback on the survey app from Samuel: "Have you had any problems so far accessing this survey? If so breifly describe them." : "Yes! the problem that i have found it concern the security of this page, my software of protection had to notify me that, and an other problem on the style sheet of this survey, the style of buttons don't make effect when it actived or focused. I have clicked the button of ‘read-home-part1’ but i haven't seen result instead i bo to the bottom page and the last problem is about the size of this questions's modal"

I think the comment about the security is because I have not installed a certificate, so some browsers warn about that. I've seen it to. It's on my todo list.

Thanks for notifying this feedback, I've been working on those 3 issues yesterday night, and the result will be seen when Ash pulls and publishes my codes updates.
