Drop the modal window and revert back to the survey on a separate page -... #3

Daniel enlarged the modal window, but we realised that the window was creating a problem: the skip button of the modal w. was appearing on the right hand side, while the submit button of the form remained on the left and was much smaller - this would have been likely to lead people to 'skip' rather than 'submit' by mistake. In addition the survey was still not fitting the modal window. We can go with the separate page for now and launch like that. Then we can explore this issue further via usability testing (but not on the live platform) and implement the solution that turns out to be the most user friendly while maximising response rate.
