It appears that the portal content on this page no longer represents the current functionality in the product. The portal now allows (or appears to allow) app name to be separated from the geolocation map, and allows both inclusions and exclusions. This page (along with https://docs.microsoft.com/en-us/azure/active-directory/authentication/how-to-mfa-additional-context) do not reflect the new interface or describe the feature changes.
