Adding camp manager association to camp #271

Fixing hidden camp crew issue by adding camp_manager_id to camp and makeing sure it is set for all camps.

Fixes #267

I couldn't find how to replicate the issue in the rspec environment, so no direct tests here, but I added a test to make sure every dream we create will have a camp_manager so hopefully won't break anything. ;-)
