MKV: Take all streams into account when calculating duration #250

Describe the issue you're having:
On files where the first video stream is shorter then the audio stream, 
calculation of the duration will be inaccurate.
Instead all audio and video streams should be used to calculate.

How can the issue be reproduced? Sample File?
