MPEG-TS: start_time/duration calculation #322

Describe the issue you're having:
MPEG-TS frequently gets the duration/start_time wrong because of different 
programs and broken streams. The solution right now is to re-calculate this 
from the current program without the subtitle/data streams, but this solution 
is ugly, and should be covered in ffmpeg, ideally.

How can the issue be reproduced? Sample File?
Locally collected samples.
