Quite a few changes here, mostly refactoring old code and adding a plugin toggle convar.

Major code cleanup across all files
- Updated code formatting to be consistent
- Updated function names to be consistent
- Moved ConVar-related code to a new file
Added a plugin toggle mechanism
- Controlled by new convar: mvm_enable (def. 1)
Starting currency may now be changed mid-game
- Player currency will be updated accordingly
