This PR adds the bulk of the P2I standard, following discussions in the working group. Rationales are currently missing.

Unanswered questions:

[ ] How do CAs share what accounts have already been used or not? The hash of the account IDs should be made public, but it is currently undefined where it should be stored.

Considering that MMPA decided it wasn't worth collaborating, I'm retracting this contribution.
