Fixed issue with Ridley due to missing name in dotdeb cookbook. Added omnibus installer. Removed mysql default recipe, doesn't exist anymore.

Hi Tom,

Thanks for the pull request! But please remove the Zend Guard Loader as I don't believe it belongs here.

In addition the README file should have Omnibus listed as a requirement and these lines removed https://github.com/MiniCodeMonkey/Vagrant-LAMP-Stack/blob/master/Vagrantfile#L47
