This fixes #472.

It tries to improve inline documentation to make using the plugin more clear.
Several plugin screenshots were out of date and they're newly generated. It also includes an updated FAQ entry in the readme.
Many documentation entries are updated, both due to this release's changes and to past changes that hadn't been documented.
