Read PC-FX metadata from disc #71

Publisher, mostly. Will need to maintain a table to translate from Japanese and otherwise keep consistency. Would be better to use maker ID, once I figure out how to work with that. Also, date. Country is just Japan, really, but check for that being 1 anyway, just out of principle. Not much else there of use, other than if volume number is actually disc number, and if I can figure out how to detect PC-FXGA-only games that'd be good (Mednafen doesn't seem to like those; and for MAME if the pcfx driver worked anyway it'd need to know to use pcfxga instead).

The stuff is there in ROMniscience, just remember that PC-FX discs are assholes and can have the header on any of the data tracks, and you have to find it yourself. And it seems to not be in the first sector either. I don't really wanna deal with that "INDEX" command in cue sheets so I'll just do the thing that way.

...Bleh. I'm gonna rewrite cd_read before I attempt this. Otherwise it's just too weird.
