I have had to do the audit2allow steps multiple times to get this working with SElinux enabled.
Each time would clear one permissions error and then reveal another. Eventually it would then start working once I got through all of them.
You may want to mention that in your Docs regarding SELinux.
Increasing debuglevel to 5 in zabbix_agent.conf will show you the errors occurring.
