Description

Note that before this revision the repository was not supporting Zabbix 4.0 LTS anymore. With this change we support both again.

The license is the same but was just converted from plaintext to markdown from the official source:

https://www.gnu.org/licenses/old-licenses/gpl-2.0.md

I didn't reinvent the template for Zabbix 4.0 LTS but I recovered it from this revision:

The current XML was just renamed to have the suffix ..._v5.xml to avoid any confusion with the v4. The README was updated to reflect this change.
