Fix a few small QCFractal server issues #685

Description

3 small(ish) fixes:

Adding a duplicate calculation that is already completed can result in the task being duplicated. This PR prevents this and also cleans up the DB
Allow larger client bodies in Tornado
Enable more efficient searching of results when multiple fields are specified
