Best would be ensure FPS camera attached to head bone or do so at death time, but ragdoll the player and fall to the ground, slowly fading to black with eye closing stencil and silence or tinnitus, maybe allowing very slow heavy-feeling mouse movement of head.
OLD WAY FIXES

Also, I noticed that camera should go straight up or it'll noclip walls sometimes.
If you die in a doorway, you'll still see out of the world.
At least one player suddenly died and wasn't aware that's what happened :)
OTHER IDEAS
My original idea was simple black screen on death, and that's still not wrong.
Could also play another sound and cut to the face of what killed you, if was a creature.
