Auto-ducking when standing or crouched a little bit will be part of the new player controller. This would help dramatically for the tall-helmeted factotum, btw.
As a separate 'not-right-now' thing, when crouching and we have crawling, should auto go to a crawl if crouching and the space only allows crawling (but again, only if already crouching)
If auto-lower to crawl, auto return to crouch when headroom available
