Number of Chapters #94

Hi, 

I have some problems with this template, first the index dos not align the 
number page, and second I try in many ways to get number of the chapter, but 
nothing, and when I try to enumerated figure and table, does not count chapter 
either, and always show chapter 0. Please helo solve my problem.
