Survey123 and Collector Templates #34

@rcarmichael3 is going to provide the data entry templates for both Survey123 (used to collect the OTG data) and Collector and/or information on how to log into our account and access that info so that we can change variable names or improve templates, if needed.

Also, might be worthwhile to include those templates in the repo, maybe inst/templates/ so that they are included during package install.

@rcarmichael3 What do you think is the easiest way to track any future changes to the DASH on-the-ground survey template? First, it would be helpful if Kevin or I (or similar) could access to edit if ever needed (e.g., to make sure column [variable] names matched those in the repo) and also, to make it easy to share the latest templates or survey forms to a collaborator.

I was hoping the template could be exported as a .csv or similar that a collaborator could upload to their account, but that doesn't seem to be the case. It appears ESRI only allows you to share as a link or QR code, which is maybe okay. Also, it appears you need Survey123 Connect to edit a template? Is that right?

Similar for the Collector form for delineating channel units? Is there a template for that, and if so, where does that reside? How could we access or share to a collaborator?

Also, should add instructions for creating, using, saving, etc. Survey123 survey templates for DASH in the DASH repo.

The Survey123 template, "DASH_MasterV6.xlsx" was added to the repo on 2/9/2021 in inst/survey_templates/
