More than 3 attributes in Forgotten User Name truncates to first 3

What steps will reproduce the problem?
1.Install pwm 1.5.5
2.Add 2+ additional attributes to the Forgotten User Name module
3.Check Forgotten User Name page - only first 3 attributes are listed

What is the expected output? What do you see instead?

Expected to see all attributes

What version of PWM are you using?

1.5.5

What ldap directory and version are you using?

eDirectory
