I think we can hit a happy medium and include some commonly-requested fields. Beyond that, need to provide documentation for site admins to manage their own custom fields.

What "custom" fields do we add?
Plain text field for a "tell us about your account request"
Do we support group membership requests? ONLY if notifications work properly.
Add a "User information" HTML block at the top of the page with some generic text for admins to update.
