Purpose of the study You are invited to participate in a survey study by the University of Amsterdam. Participation is limited to adults (18 years of age and older). The purpose of this study is to understand if our approach can empower citizens in a local region in the Netherlands to monitor air pollution. This survey will take you approximately [X] minutes to complete. The data will be used to derive empirical design implications that can guide the development of our citizen engagement tool.

Study procedure You will be asked to fill out questionnaires. Please avoid putting personal identifiers in the questionnaires.

Benefits and risks of participation The risks associated with participation in this study are no greater than those ordinarily encountered in daily life. There may be no direct benefit to you. However, the research results that come from your data can contribute knowledge to humanity.

Procedures for withdrawal from the study Your participation in this study is entirely voluntary and you can withdraw at any time. You are free to omit any question. ~If you put your contact information in the questionnaires, you can contact us within 3 days after the study to retract your data. After 3 days, we will anonymize your data by destroying the link between your contact information and the questionnaires, and it will be impossible to remove the data anymore.~ If you wish to withdraw your consent after the study, you can contact us (with the participant ID that we give at the beginning of the survey), and we will remove your data. However, if the data is already processed into insights, submitted to a venue for review, or published (e.g., in research papers, technical reports, online articles, academic presentations, etc.), we will not be able to remove these processed data.

Data collection and processing We will not collect personal identifiers. We will process the anonymous data you contribute to the questionnaires.

Data usage and privacy The analysis results and the data collection methodology may be published in research papers or technical reports. We will aim for open-access publication. During the active phase of research, the project leader will oversee the access rights to data (and other outputs), as well as any requests for access from external parties. Sharing of data with others will only be done in such a manner that participants will not be identified. We will enter the data into digital forms and host them on password-protected storage. The paper survey will be kept in secure and locked physical storage at the University of Amsterdam. The physical paper survey will be destroyed after the research is done, and only the digital version will be kept.

Research participant rights If you have concerns or questions about this research study, or if you believe that you have been harmed due to participation in this study, please contact the researchers, listed at the beginning of this informed consent form.

Provide your consent I have read and understood the study information. I consent voluntarily to participate in this study and understand that I can refuse to answer questions, and I can withdraw from the study at any time without having to give a reason. I understand that taking part in the study involves taking questionnaires. I understand that the information I provide will be used for data analysis, and the anonymous data and the results may be released publicly in research outputs, including academic papers, technical reports, and online articles. I agree that the data I provided in the study can be quoted in research outputs. I give permission for the non-identifiable data that I provide to be archived into an anonymized dataset so it can be used for future research and learning.
