On windows the error-report form has the Java coffee cup as the icon #662

As per title. It would look nicer if it used the same icon as MBHD itself

Added our icon to the frame.

Ready for review and close.

Built an installer and tried it out on Windows 8. The 'Report error' screen now uses the MBHD icon. Closing.
