Increase the maximum fee to 2.0 mBTC per KB #956

According to https://bitcoinfees.21.co/, it's currently only possible to have your transaction scheduled within the very next block by paying a fee of at least 1.2 mBTC per KB, but the current maximum is set to only 1.0 mBTC per KB.

This PR bumps up the maximum to 2.0 mBTC per KB, as well as fixing up some of the comments.

This is incomplete. KeepKey need to perform a review of this to assess the full impact of making a change of this nature.
