Changes to the Perl HTML generating functions

I know this seems like (maybe is) a lot of changes, so let me break it down:

• Change backtick functions Perl isn't portable with any backtick functions. Use Perl equivalents.
• _Use MUTOPIAWEB File writes and reads need to use the MUTOPIA_WEB environment variable.
• MSDOS line endings musiccache.dat and its temp file need to use Linux line endings or the seek will be off.
• Error reporting I changed the die()s to croaks()s. The problem with using die for error reporting is you can't see where the function was called from. croak fixes that.
• Formatting There were two or three places where the formatting was just wrong and you couldn't see the control structures correctly.
• htdocs I put all the HTML files in a folder called htdocs under MUTOPIA_WEB and added it to .gitignore. This can be changed if necessary.

I had to use ":raw:utf8" to get the script to work on Windows. :raw is a "layer" that strips off all other layers (like :crlf), then you add :utf8 on top of that. See if this works in Linux.

Verified on my Linux machine. My "basic" test is to build the files, deploy to the server, and invoke make-table by selecting an list by instrument, then a few advanced searches.
