Recreate this module with Tripal HQ #54

Write a MD guide on building this module with Tripal HQ.

Here is my organism form (which an end user would use, thanks to tripal HQ).

We can also add back in the "Thanks for submitting" text block at the header if we want it.

I removed a bunch of questions which i inferred were no longer wanted, but if we want to add back in let me know.

Here's the old one for reference:

note users are required to make an account, so the user info is not necessary.
