Shared scheduling goals/conditions UI MVP should include the following:

### Tasks
- [ ] Should remove model/plan specification from goals and conditions creation form
- [ ] Should add a way to delete goals and conditions versions
- [ ] Should add a place in a plan to associate goals and conditions with viewed plan
