When constraint violation windows are < 1px wide they are not visible in the timeline. Suggestion is to force a 1px min width for all constraint violations and/or explore some form of icon in the area where the plan simulation icons live.
Reproduction
Make a constraint violation that has a very small window and note that it is not visible in the timeline unless you zoom in or click on the constraint violation window from the constraints panel.
