disable cognito-dependent tests and extend flake8 lint ignores

🗒️ Summary

Disables four (important) test cases which fail when run in an environment that doesn't include a jwt/cognito-populated config.
Add a tonne of flake8 ignores
Add a mypy type ignore for external module jose which does not provide type hints in the current version.
Renames tox env py39 to tests
Fixes errors in documentation regarding running individual tox environments

@nutjob4life sorry to bug you for a second one, but I was hoping you might cast your eye over the tox/doco changes to sanity-check them in case there's a good reason they were the way they were.

I've also added Branch Testing to the (previously empty) list of status check which must pass to merge a PR.
