Ownership Info not passing through OIG #45

I entered a known excluded individual as sole owner of a clinic, but the EXCLUDED PROVIDER VERIFICATION IN OIG in Review Application is checked, as though everything passed, and the SSN DMF VERIFICATION does not have a value.
