misc dataset form enhancements #8

[ ] remove checkboxes from 'all' links in 'Variables' section
variables
- [ ] move checkboxes to left of names
- [ ] add space between checkboxes and names
don't link pages to themselves
- e.g. http://datavis.eol.ucar.edu/ncharts/projects/Weather/flab-5min
