There are some things that are in both mondo and HP. (Say, ALS, or agoraphobia). We've wanted to keep those as separate entities. The problem is that things like UMLS or MESH might join them together. So transitively they're the same thing.
I think we should unify all of these cases, but whether we unify or separate, we need to clean up disease/phenotype handling in the synonymizer to handle this case.
