Change Salutation to multi-select dropdown #55

Change the 'Salutation' field for Contacts to be a multi-select dropdown containing 'Mr', 'Mrs', 'Ms', and 'Dr'.

Closing. Using the full name is easier than gendered pronouns, and avoids errors in greetings.

Can re-add "Dr" if we need it for professional reasons.
