Knitted still points to term server. FHIR items not retired or amended re retirement on code 01

Note also that MICROSCOPIC_INVOLVEMENT_INDICATOR_FOR_VAGINAL has been detailed as a concept clash...but cannot see the clash in sept or august release?

De-prioritised - not being looked at until branch migrations enabled

From Maxine 28/1/22: Put these in the backlog for now. Likin has been prioritised towards enabling branch migration and CR migration ahead of any tickets re FHIR gen history. Ideally – these will be addressed pre migration – Only post migration if we really have to.
