NIAD-000: Update pathology record naming. #85

Description

As the adaptor will handle pathology and screening messages, we should rename the pathology record model and mapper to "MedicalReport".

Also removed the unused "SpecimenCollector" and "SpecimenCollectingOrganization" properties.
